Casework Service

本處沙田及大埔綜合家居照顧服務中心為區內缺乏自我照顧能力的長者、殘 2020-2021
疾人士或有需要的家庭提供到戶的綜合家居照顧服務，使他們能夠達成「居
家安老」的心願。在嚴峻的新冠肺炎疫情下，本處的同工仍全力以赴，繼續
為社區上的長者及護老者提供綜合家居照顧服務支援。兩個服務單位於本年
度合共處理了五百三十四宗普通個案及一百五十一宗護理個案。

沙田及大埔綜合家居照顧服務中心因應服務對象的不同需要，為他們提供：
個人照顧、簡單護理、家居清潔、護送服務、購物及送遞服務、膳食及洗衣
服務、家居安全及健康評估服務、日間到戶看管、照顧幼兒及護老者支援服
務等普通個案服務；經社會福利署安老服務統一評估機制定為身體機能中度
或嚴重受損的體弱長者，中心則按照服務使用者的需要提供：醫療指導、營
養指導、物理治療、職業治療、言語治療、照顧者支援服務、日間暫託及住
宿暫託轉介和二十四小時緊急支援服務等護理個案服務，讓長者能實踐「居
家安老」。

The Shatin and Tai Po Integrated Home Care Service Centres of the Agency provide
Direct-To-Home integrated home care services to elderly who lack self-care ability,
persons with disabilities, or families in need for fulfilling their wishes of “ageing in
place”. The two service units have processed a total of 534 ordinary cases and 151
frail cases this year. Under the severe COVID-19 outbreak, the staff of the Agency has
made every effort to maintain the integrated home care services to the elderly and
carers in the community.

The Shatin and Tai Po Integrated Home Care Service Centres provide services for
ordinary cases according to their needs, ranging from personal care, simple nursing
care, household cleaning, escorting, purchase and delivery, catering and laundry,
household safety and health evaluation, home respite service, child-minding and
carer support service. The Centres also provide services to frail cases serving elderly
whom were assessed to be in state of moderate or severe level of impairment by
the Standardized Care Need Assessment Mechanism
for Elderly Services implemented by the Social Welfare
Department. The services include medical consultation,
nutrition consultation, physiotherapy, occupational therapy,
speech therapy, carer support, centre-based day care,
respite referral and 24-hour emergency support, enabling
elderly to “age in place”.
