本年度，沙田綜合家居照顧服務中心繼續參與關愛
基金的「支援身體機能有輕度缺損的長者試驗計
劃」，旨在為經評估為身體機能有輕度缺損的低收
入長者提供所需的家居照顧及支援服務。中心與符
合資格的長者商討，並為其制定個人照顧計劃及服
務收費，以及盡快提供所需的膳食服務及／或家居
服務等支援服務，包括：個人照顧、簡單護理、一
般運動、家居清潔、護送服務、購物及送遞服務
等。中心於本年度共處理了七十一宗個案。大埔綜
合家居照顧服務中心則繼續協助轉介四十八位有需
要的長者參與此項關愛基金服務。另外，兩間中心
亦繼續協助轉介有需要的長者參與關愛基金牙科資
言語治療服務

Throughout the year, the Shatin Integrated Home Care Services Centre continued to participate in the “Pilot Scheme
on Home Care and Support for Elderly Persons with Mild Impairment” of the Community Care Fund, which aimed to
provide home care and support services for low-income elderly persons with mild impairment. After communicating
with the eligible elderly, the Centre would formulate a personalized care plan with service charge and provide the
needed meals and / or household services at the soonest, including personal care, simple nursing care, general
exercising, household cleaning, escorting and purchase and delivery services. The Centre has processed a total of
71 cases this year. Tai Po Integrated Home Care Services Centre continued to assist in referring 48 elderly persons in
need to join this service organized by the Community Care Fund. The two centres would also keep on referring elderly
in need to participate in the Community Care Fund Elderly Dental Assistance Programme.
