                                                                of preventive fall. The 50 elderly people who were assessed
                                                                as high fall risk, social workers and occupational therapist
                                                                visited individual households to provide further assessment
                                                                and follow up work such as applying for electrical appliances,
                                                                allowance from building maintenance grant scheme and long-
                                                                term care services, so as improve their living condition.
















            「愛•在家」體弱長者居家支援計劃                                    “LOVE@HOME”
            為加強提供護老者及認知障礙症社區支援服務,本中心在                           Home Support Scheme for the Frail

                                                                improved their physical and mental health to enhance their
                                                                sense of self-efficacy.
