(1) 本人謹此聲明在本申請表填報的資料均正確無誤，並授權職業訓練局（職訓局）使用這些資料，作為處理本人課程申請之用。 亦

I declare that the information given in this application is, to the best of my knowledge, accurate and complete. I authorize
V V Vocationalocational Training Council (VTC) to use the information given in the admission process of the courses offered by VTC. I
understand that provision of any false or misleading information therein shall lead to DISQUALIFICATION of my application without
notice and cancellation of any resultant registration.
(2) 本人明白申請表上所填報的資料將作如下用途：
I understand the information given in this application is used in the following purposes:
i. 處理一切有關 2016 至 2018 年度職訓局應用學習課程入學的申請事宜；
in processing the application for VTC Applied Learning Courses for 2016/18 cohort; and related purposes;
ii. 用作申請報讀參與以上課程的甄選和其他相關用途；
in selecting applicants for admission;
iii. 本人如獲取錄，資料將會在學生紀錄系統存檔，職訓局可利用該等紀錄作學術或行政之用；及
the application form and other related personal information will be retained upon successful admission to the courses for
student records and other purposes relating to my study in VTC, and
iv. 作統計及分析用途。

(3) 本人明白職訓局會把本人的資料保密，但職訓局可能會將本人有關資料提供給任何對職訓局有保密資料承諾的人士或其代表作為列舉

I understand that the information will be kept confidential by VTC. However, VTC may provide such information to any other
persons or agents for the purposes described in (2) under a duty of confidentiality to VTC.
(4) 在一般情況下，未獲取錄申請人的資料將於 2016 至 2018 年度職訓局應用學習課程入學程序完成後全部銷毀。
Information on unsuccessful candidates will normally be destroyed after the completion of the application for VTC Applied Learning
Courses for 2016/18 cohort.
(5) 根據個人資料(私隱)條例，本人有權查閱職訓局是否持有我的個人資料，並要求獲得有關資料的複本。本人亦可要求職訓局更正有關
我的個人資料。本人如欲查閱個人資料，必須以書面向職訓局應用學習組提出（地址：香港灣仔活道 27 號職訓局大樓 1318 室），並
提供足夠資料以識別身份，否則職訓局有權拒絕本人查閱資料之要求。職訓局亦保留權利收取查閱資料的費用。
In accordance with Personal Data (Privacy) Ordinance, I have the right to check whether VTC holds my personal data and request
a copy if needed. I have the right to request VTC to correct my personal information, if needed. I shall write to VTC Applied
Learning Team (Address: Room 1318, VTC Tower, 27 Wood Road, Wanchai, Hong Kong) for enquiry on my personal data, with
sufficient proof of self-identity to VTC. Otherwise, VTC shall reject my request. VTC reserves the right to collect fees on the
enquiries concerned.
